Center for Human Services (CHS)is seeking supportive and personable professionals to join our Behavioral Health Team as Case Managers. These vital roles based in Stanislaus County involve working with children and their families at designated school sites offering support and guidance during regular school hours from Monday to Friday. Candidates must be bilingual in English and Spanish with the ability to read speak and translate fluently. POSITION SUMMARY:Case Manager will support referred students and their families to assess client needs access available resources and develop a plan for reaching goals. This position partners with assigned schools and is part of an integrated multi-disciplinary service team. Case Manager is expected to provide screenings facilitate meetings and meet with clients and their families at assigned sites or within the community

DUTIES:

  1. Complete screenings for clients referred to school based Medi-Cal program and communicate outcomes and complete appropriate follow up care.
  2. Assists clients and their families in accessing needed medical educational social rehabilitative and other community services through coordination communication and referrals.
  3. Develops individual service plans for each client to monitor clients progress in reaching service goals.
  4. Engage clients in developing age appropriate daily living skills aimed at enhancing clients self-sufficiency or self-regulation.
  5. Schedule and facilitate Child Family Team Meetings (CFT) for identified clients within established timeline and complete all required documentation.
  6. Consult with clients family school staff and other care providers with clients consent to education them to help client achieve self-sufficiency or self-regulation in multiple life domains.
  7. Meets regularly with clients in the field at homes schools established Mental Health Department and CHS sites in Juvenile Hall and other appropriate sites.
  8. Transports clients in Agency owned vehicles as needed and approved.
  9. Interacts collaboratively within a multi-disciplinary team and supports team members in meeting client needs.
  10. Assists with contact liaison and development of close working relationships with agencies schools businesses etc.
  11. Complete all necessary charting reports and statistical information necessary for assigned client population promptly.
  12. Meet contractual obligation by billing a determined number of Medi-Cal reimbursable hours.
  13. Provide interpreting services for mental health assessments as needed or other contacts with families at the request of clinicians.
  14. Other duties as assigned.

QUALIFICATIONS:

  1. High school diploma/GED required; BA/BS preferred.
  2. Bilingual - English/Spanish required. Ability to speak read and write in Spanish fluently.
  3. Two years experience in human service field such as crisis intervention group and individual support services or working in the mental health field
  4. Experience working with electronic health record or similar database system.
  5. Experience and ability to work effectively with people of all socioeconomic and cultural backgrounds.
  6. Effective communication problem-solving critical thinking and interpersonal skills.
  7. Case management or coaching experience preferred.
  8. Experience working in a clinical or mental health setting preferred.
  9. Pass agency paid criminal justice screening including fingerprints.
  10. Pass agency paid TB (Tuberculosis) screening.
  11. Pass agency paid health screening and/or drug testing. (if required)
  12. Valid California drivers license and proof of auto insurance coverage.
  13. Pass agency paid MVR clearance with no more than 2 points - if you are under the age of 25 our insurance company will not allow any points.

  3. Establish and maintain effective working relationships with the general public co-workers clients supervisors and members of diverse cultural and linguistic backgrounds regardless of race color creed religion gender sexual orientation gender identity or expression national origin age ancestry political affiliation citizenship disability medical conditions marital status amnesty and military or veteran status.
  4. Will promote and support a culturally and linguistically diverse workforce and be responsive to the population within our service area.
  5. Maintain confidentiality and confidential information in accordance with legal standards and/or agency regulations.
  6. Participate in assigned scheduled agency meetings in-service trainings conferences and other trainings as determined by the supervisor. This includes serving as an agency representative at assigned community meetings.
  7. Observance of assigned working hours and program appointments by demonstrating promptness and thorough preparation.
  8. Performance of assigned duties with a positive attitude and in the spirit of teamwork collaboration and cooperation.
  9. Communicate effectively both orally and in writing.
  10. Perform job duties in a safe manner to ensure a safe working environment for oneself and others.
  11. Participates in and/or supports agency fund development activities and events.
  12. Preparation of assigned reports work records statistical data job performance evaluations work plans etc. in a timely manner.
