Position Summary
The Department of Psychiatry at the University of North Carolina at Chapel Hill School of Medicine is seeking a Social Services Professional to serve as Health & Wellness Coach for an innovative multidisciplinary team addressing many health disparities in young people with psychotic disorders. Responsibilities for the Health and Wellness Coach include: Help people adapt to healthier lives by discovering strengths motivations and skills that will enable them to change their mindsets and behaviors. Have regular contact with clients to assist them in constructing attainable goals offer support and encouragement track successes and setbacks and provide resources tips and strategies to facilitate ongoing progress. Use a holistic approach to wellbeing integrating exercise/activity levels weight management and tobacco cessation skills into a healthy living model using wellness wheel components: physical intellectual spiritual environmental occupational social and emotional. Be an integral part of each clients multidisciplinary care team. You will collaborate with a team of psychiatrists therapists and peer support specialists to help develop and reinforce clients progress toward their health goals. The frequency of client meetings will be determined based on individual needs and discussion with the rest of the team. The sessions will be virtually and may include video phone and text. Have weekly supervision by the team psychiatrist. Provide one-on-one coaching on various health topics such as weight management increasing physical activity and smoking cessation. Implement coaching plans using behavior change principles to identify client barriers and develop ways to overcome those barriers. Maintain accurate and timely documentation in the electronic medical record. Collaborate through team meetings and notes with clients clinical teams.
Required Qualifications Competencies And Experience
Must be certified by the National Board of Health and Wellness Coaches or hold an active NC license in a medical field (e.g. Registered Dietician Recreation Therapist Occupational Therapist or Licensed Clinical Social Worker/Licensed Mental Health Counselor). Must have experience using coaching techniques such as Motivational Interviewing SMART goal setting and assessing stages of change.
Preferred Qualifications Competencies And Experience
Experience working with young adults with mental illness is preferred.
