Overview
Location: Grand Junction IA area
$10000.00 New Hire Sign-On Bonus!
Shift: Monday - Friday 7:30 AM - 4:30 PM
Join a compassionate team dedicated to delivering high-quality one-on-one care to pediatric through young adult patients with special health care needsright in the comfort of their own homes. As a skilled nurse youll provide personalized care under the direction of the patients physician including assessments medication administration respiratory support feeding and assistance with daily living activities. Youll play an essential role in implementing therapy programs and collaborating with the broader care team to ensure holistic support. In some cases you may also assist with patient transportation. Responsibilities
Delivers quality patient care consistent with Home Health Conditions of Participation qualifying home care criteria payer coverage criteria and providing visits/treatments consistent with the plan of care.
Communicates with Registered Nurse Case Manager/Clinical Supervisor and participates in care coordination to achieve high quality care.
Performs other duties as requested by the Clinical Manager/Clinical Supervisor to facilitate smooth and effective operations of the organization.
Maintain accurate and timely documentation of clinical records and time entry.
Monitors the patient care process to evaluate and improve care collaborating with providers and other health care workers to provide comprehensive health assessment treatment and follow up evaluation.
Qualifications
Education:
- Graduate of State Board approved program for Registered Nurses
Licensure/Certifications:
- Current license to practice nursing according to the state(s) where service is provided
- Maintenance of an acceptable driving record (i.e. no OWI convictions in last 7 years or no more than 3 moving violations in last 3 years)
- BLS
