We are currently seeking a Wound Care Nurse to join our skilled nursing team at The Grand Rehabilitation and Nursing at Barnwell.This vital role focuses on the prevention assessment treatment and management of wounds ensuring optimal healing and improving the quality of life for our residents. You will work collaboratively with physicians nursing staff and interdisciplinary teams to deliver top-quality wound care while educating staff and residents on proper wound prevention techniques.
Perform wound assessments and create individualized care plans.
Provide direct wound care treatment using evidence-based practices.
Monitor healing progress and adjust care plans accordingly.
Document wound care in compliance with regulatory standards.
Educate nursing staff on wound prevention and treatment best practices.
Collaborate with physicians and interdisciplinary teams on wound care strategies.
Ensure timely referrals for specialized wound care services when needed.
Maintain infection control protocols and regulatory compliance.
Valid RN or LPN License in the state of New York (Required).
Certification in Wound Care (WCC CWCN or similar) preferred or willingness to obtain.
Experience in wound care or long-term care setting preferred.
Strong clinical assessment documentation and communication skills.
Ability to work collaboratively and independently in a fast-paced environment.
