Job Description

SummaryWe are seeking a Community Outreach Specialist to join our dynamic team at S&B Consulting Group. This role is pivotal in enhancing community engagement and fostering relationships that align with our mission of connecting exceptional talent with outstanding ● Develop and maintain relationships with healthcare professionals and community partners● Collaborate with Enrollment Specialist to ensure successful enrollment● Organize and conduct events to promote the PACE program● Maintain CRM software records● Adhere to DHCS and CM regulations● Develop marketing plans for enrollment● Collaborate with management on retention strategies● Attend community and healthcare events to increase census● Identify opportunities to enhance referral experience in volunteer management or community outreach project management skills with the ability to handle multiple initiatives in program development and implementation is communication skills both written and verbal for effective community in public health or social work is a plus but not and Shift Details: 8a-5p M-F● Hybrid - In field work 2-3 days/wk onsite at clinic center 2-3 days/wkTravel - Local to LA areaBilingual Requirement: English ● Spanish
