Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with an interdisciplinary team. Strong communication and collaboration skills are essential.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with interdisciplinary team (physicians other nurses therapists etc.).
- Case management and coordination of care.
- Providing skilled nursing care in various settings (hospital home health hospice).
- Maintaining accurate patient records and documentation.
- Responding to patient needs and concerns.
- Participating in patient education and discharge planning.
Required Skills & Certifications:
- Clinical assessment medication administration and treatment planning skills.
- Active Registered Nurse (RN) license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of experience in hospital home health or hospice care.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and personal automobile for field work.
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- Rotating weekend coverage is required.
