POSITION TITLE: Caseworker I (IDD Access and Intake Service Coordinator)
Duties include screening of individuals requesting Center services providing referrals to more appropriate community resource and/or making referrals to appropriate Center services. Facilitate the DID process and the Center admission process for new consumers. Represent IDD services at annual ARD meetings with School Districts within the service area to facilitate transition services. Coordinates and assists the Continuity of Care Service Coordinator with the maintenance of the HCS Interest List and enrollments into the HCS & ICF/ID programs and admissions to the State Supported Living Centers (LLSC). May at times provide Service which includes assessment treatment planning monitoring of services and crisis prevention and management. Develops and maintains referral resources for service provision allowing for consumer choice where available of providers. Authorizes provision of services based upon assessed need and availability ensures best value of resource allocation to service provision.
EDUCATION TRAINING & EXPERIENCE
- A bachelors or advanced degree from an accredited college or university; or
- An associate degree in a social behavioral human service or health-related field including psychology social work medicine nursing rehabilitation counseling sociology human development gerontology educational psychology education and criminal justice; or
- A high school diploma or a certificate recognized by a state as the equivalent of a high school diploma and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
ESSENTIAL TASKS
Crisis Services
- Available for after-hours consultation.
- Provide crisis services as needed 8:00 a.m. to 5:00 p.m.
- Monday Friday and on rotation basis for after-hours holidays and weekends.
- Respond to all crisis calls within 10 minutes; 30 minutes if face-to-face assessment is needed.
Intake Services
- Completes screening registration paperwork.
- Prepares IDD Admission Plan and related paperwork.
- Screens phone calls and walk-ins of individuals requesting IDD services.
- Coordinates the DID process for individuals requesting IDD services.
- Coordinates the admission process for individuals requesting IDD services.
Service Coordination
- Complete all PDPs and PDP reviews on or before expiration date ensuring the document is signed by client and responsible staff with client receiving a copy.
- A Person Directed Plan is completed that incorporates the comprehensive assessment and the individual choices of the client including family members and support systems at the clients discretion.
- PDPs identify client strengths problems/needs objectives/goals
- strategies and individual preferences as well as areas for improvement.
- PDP and Quarterly Reviews are monitored to assure each consumers response to services.
- Encourages participation in the PDP process.
- Participate in Medicaid time studies to determine how time is distributed across a range of functions and activities performed by this position with minimal error.
- Provides face-to-face contact monthly or as indicated but no less frequently than every three months with assigned caseload.
- Complete progress notes within 24 hours of service provision.
- Progress notes record progress towards outcomes in the PDP and other clinically significant events. Therapeutic language is used in all instances of clinical record documentation. Discharge summary is completed and entered into record within 30 calendar days following discharge.
- Compliance with Medicaid and TDADS documentation standards.
- Clients report an 80% or greater satisfaction rating for services provided.
Continuity of Care
- Provides unbiased choice of providers when facilitating the enrollment of individuals into community residential programs. Assures that proper documentation and assessments are completed for individuals moving to a residential setting (i.e.: PDP IPC ID/RC and the ICAP)
- Participates in staffings as LA representative in placement deliberations for Individuals from our catchment area or individuals seeking placement in our catchment area.
- Monitors clients as needed to ensure continuity of care in the least restrictive environment.
Assures at least 50% of the individuals on the HCS /TxHmL services interest lists are contacted within the first year of the FY contract and the other 50% are contacted the second year of the FY contract. (100% contacted within two years
REQUIREMENTS
- Valid Texas Driver License required.
- Must not have more than 3 moving violations in the last 3 years.
- Must be insurable on the Centers insurance.
- Must be able to pass a background check.
- Must be able to complete all MHMR Concho Valley training.

Monday through Friday 8 a.m. 5 p.m. days and times may vary.

$2680.65 - $3155 monthly
