POSITION TITLE: Caseworker I (IDD Access and Intake Service Coordinator)
Duties include screening of individuals requesting Center services providing referrals to more appropriate community resource and/or making referrals to appropriate Center services. Facilitate the DID process and the Center admission process for new consumers. Represent IDD services at annual ARD meetings with School Districts within the service area to facilitate transition services. Coordinates and assists the Continuity of Care Service Coordinator with the maintenance of the HCS Interest List and enrollments into the HCS & ICF/ID programs and admissions to the State Supported Living Centers (LLSC). May at times provide Service which includes assessment treatment planning monitoring of services and crisis prevention and management. Develops and maintains referral resources for service provision allowing for consumer choice where available of providers. Authorizes provision of services based upon assessed need and availability ensures best value of resource allocation to service provision.
