Job Description

Summary

Nurse Practitioner or Physician Assistant

General Summary

This position provides patient care for adult patients in the outpatient Cardiology Clinic setting with a focus on general cardiology. Additional scope and responsibilities may include advanced heart failure pulmonary hypertension and preventative cardiology.

Full time 1.0 FTE position

Expectations include 32 hours of patient facing clinic time per week and 8 hours per week of non-patient facing time allocated for administrative tasks associated with provision of clinical care.

Essential Responsibilities

  • Provides care for adult patients in the Department of Medicine cardiology clinics with a focus on general cardiology.
  • Additional scope and responsibilities may include advanced heart failure pulmonary hypertension and preventative cardiology.
  • Performs new and follow-up visits gathering data from initial evaluation develops and documents an assessment and plan of care.
  • Monitors progress of assigned patients and revises orders and care plans as needed. Identifies problems that may need special attention or more acute care. Refers for testing and specialty consultation as indicated.
  • Generates progress notes with each outpatient visit. Maintains communication with referring providers.
  • Assists medical team in determining a home care plan for patient and provides appropriate care.
  • Makes professional contributions to the Department and Division through engagement in committees research projects or other scholarly work.
  • Performs all other duties as assigned.

Travel
Travel between NJH sites of practice for APPs as needed.

Minimum Qualifications

  1. Education: Graduate degree from an accredited Physician Assistant or Nurse Practitioner program required.
  2. Work Experience: 1 year of recent and related experience as an Advanced Practice Provider preferred.
  3. Special Training Certification or Licensure: Colorado PA license or APRN license and prescriptive authority. DEA Registration. PA or NP certification by a national certifying body. Current CPR/BLS certification is required. ACLS or PALS certification if applicable for specific practice area.

Salary Range:$105000 - $135000
