Job Description

Job Title: Family Peer Advocate

Department: School Based Program

Location: John James Audubon School #33 500 Webster Ave Rochester NY 14609

SUMMARY:

The Family Peer Advocate will support families and caregivers with children in the Youth and Family department by offering guidance resources and advocacy based on shared lived experiences. The advocate will assist families in accessing services understanding their rights and connecting with community resources. They will work closely with healthcare professionals social workers and other agencies to ensure that families receive the support and services they need.

RESPONSIBILITIES:

  • Provide informational and educational support on system navigation advocacy and rights to empower families to become active participants in their childs services.
  • Make referrals and linkages to needed services and resources and assist with access.
  • Support the efforts of families in caring for and strengthening their childrens mental and physical health development and well-being of their children by assisting the family to learn and practice strategies to support their childs positive behavior.
  • Facilitate parent training and education to increase knowledge and skills while providing opportunities for networking with other families and creating natural supports.
  • Form and maintain relationships using effective communication and collaboration acting as a bridge between family and providers.
  • Administer assessments and help family to identify gaps in services needed and assist in planning to eliminate gap.
  • Promote effective family-driven practice. Attend and/or facilitate meetings to promote family voice and infuse a family perspective at all levels.
  • Maintain all necessary documentation in compliance with RRH and OMH standards.
  • Obtain and maintain credential based on advocates role from OMH approved credentialing program.
  • Participate within a multi-disciplinary team and attend team meetings in-service trainings and team building activities as well as volunteer and serve on agency and community committees as needed. Serve as a resource to guide team performance.
  • Lead/conduct Family Peer Advocate staff training and manage projects as directed.
  • A willingness to provide services in the familys homes or alternate locations in the community.
  • Provide support to multiple sites across the Youth and Family department as determined by need.
  • Participate in regularly scheduled supervision.
  • Demonstrate strong interpersonal skills and maintains positive working relationships.
  • While this job description covers many aspects of the role employees may be required to perform other duties as assigned.
  • Travel will be required for client visits.

REQUIRED QUALIFICATIONS:

  • Provisional Family Peer Advocate Credential (PFPA)
  • Family Peer Advocate Credential must be obtained within 18 months of receiving the Provisional Family Peer Advocate credential.

PREFERRED QUALIFICATIONS:

  • High School Diploma/GED preferred

S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

$17.25 - $21.50

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience relevant qualifications specialty internal equity location and contracts.
