Job Description
Title: Occupational Therapist Home Health
Department: Therapy / Rehabilitation
Type: Travel / Contract (13 weeks)
Schedule: Day Shift 5x8-Hour (08:00 AM 4:30 PM) No weekends no on-call
Start Date: 08/18/2025
Summary:
We are seeking a qualified and experienced Occupational Therapist (OT) for a 13-week Home Health assignment in Medford Oregon and nearby communities. This is a travel position focused on 1:1 care with an average of 6 patient visits per day.
Responsibilities:

Provide skilled OT services in the home health setting

Assess and implement therapy plans based on individual patient needs
-
Collaborate with interdisciplinary team to ensure optimal patient outcomes

Maintain documentation per facility and regulatory standards
-
Travel to multiple patient homes daily within a defined territory
-
Achieve 30 productivity points per week (each visit counts based on complexity)

Utilize OASIS documentation for evaluations and recertifications
-
Must have 1 year of recent skilled home health OT experience

OASIS experience required please disclose clearly if not present

Must have BCLS certification
-
Must provide own transportation
-
Must be comfortable with occasional long-distance travel (radius up to 50 miles)

Locals accepted within 50-75 miles at the same rate
-
First-time travelers not accepted

Pending license considered must be active by start

No floating or block schedule available

RTO must be disclosed upfront and confirmed with the hiring manager
Education:

Graduate of an accredited Occupational Therapy program
Experience:
-
Minimum 1 year of recent skilled Home Health experience required

Prior experience with OASIS documentation required
License:
-
Oregon OT License (active or pending at time of submission)
Skills:
-
Strong clinical and assessment skills in home care environment

Ability to manage caseload independently

Excellent communication and organizational skills
-
Competency in documentation especially in OASIS
