Position Title: Case Manager
Department: Homeless Services
Reports to: Program Director Community Supportive Housing
Employment Status: Full-Time
FSLA Status: Exempt
Salary: $53000 - $65000 annual
The Community Supportive Housing Program provides permanent supportive housing to formerly homeless individuals and families in subsidized scattered-site apartments using the Housing First model. The program is designed to lower barriers to housing so that highly vulnerable and chronically homeless individuals and families can gain permanent housing. Supports from the Case Manager include coordination of care life skills development and tenancy support. Referrals to outside providers for assertive community treatment and other behavioral health services are also integrated with the support HIP provides. The Case Manager is responsible for a caseload of about 20 residents with whom they must meet at least twice per month.
