Catholic Charities is seeking a Case Manager. Annas House provides eight transitional living spaces and a broad array of homeless services including emergency shelter transitional housing for families fleeing domestic violence community supportive housing and rapid re-housing. Annas House provides supportive services where each family or individual begins to take charge of their life and recognizes their own capabilities and self-worth. Our goal is to provide personalized assistance and encouragement to each client as they move toward self-sufficiency. The work schedule is Monday - Friday 9am - 5pm with one evening shift 12pm - 8pm
JOB DUTIES & RESPONSIBILITIES:
- Work with clients to primarily focus on improving quality of life in obtaining stable and permanent housing goal setting case planning and personal development
- Maintain casework files on each client according to policies and procedures and organize and record information into the file and HMIS in a timely manner
- Provide support advocacy information and referral services to clients
- Participate in and accurately track case management progress for 30/60/90 team meetings in HMIS
- Motivate and empower clients to establish and reach goals including obtaining information from clients such as: clients interests aptitudes and abilities for developing case plans
- Ensures client confidentiality is maintained in accordance with HMIS guidelines and requirements.
- Other duties as assigned.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- Bachelors degree in Human Services Social Work or related field required
- Two years related work experience required
- A minimum of one year of experience working with at risk individuals
- Requires the ability to utilize computer systems and software necessary to perform position
functions. Basic Windows PC web browsing (i.e. Chrome Internet Explorer etc.) and Microsoft
Outlook skills required. Other Microsoft Office application knowledge such as Word Excel
PowerPoint Skype and OneDrive desired - Valid drivers license issued by the state of your residence with no state issued restrictions and no more than three (3) points that would impede your ability to operate a vehicle. Two years of driving experience.
PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:
- Must be physically capable of moving throughout buildings that have stairs
- Driving (Agency and/or personal vehicle) is required for this position. Driver must be at
least 21 years of age with a minimum of 2 years of driving experience and a valid drivers
license issued by the state of residence. There can be no state issued restrictions on the
licenses that would impede drivers ability to operate the vehicle as required by the
Agency and there must not be more than three (3) points on their driving record.
