About the Department
Alfred Health Pharmacy is well recognised for high quality and innovative patient- focused services education and training programmes as well as significant research output. The service operates over 4 sites within Alfred Health and continually advocates for postgraduate learning and research interests with supportive partnerships and expert mentors available to foster staff development.
The department also has a leading Digital Health strategy playing a pivotal role in managing the electronic medication management system within our comprehensive electronic medical record (Cerner) with an advanced level of integration linking this to the Pharmacy Dispensing (iPharmacy) and Automated Medication Storage systems (Omnicell). This is complemented by a strong clinical presence and a Centre for Medicine Use and Safety program to achieve a considered safe and financially responsible approach to medication management within Alfred Heath.

The Caulfield Hospital team specialises in community-based care rehabilitation and aged care services. Our services extend across hospital community and home-based settings allowing care to be tailored to individual patient needs.
Our pharmacists are recognised as essential members of all clinical teams actively engaging in a unit and team-based care model involving ward rounds and collaborating closely with the multidisciplinary team to achieve excellent patient care outcomes. There are vast opportunities to engage in Advanced Scopes of Practice. This includes undertaking credentialing programs and leading clinical activities such as Partnered Pharmacist Medicine Charting Therapeutic Drug Monitoring Nicotine Replacement Therapy and Analgesia de-escalation to name a few.
About the Role
Alfred Health is seeking an experienced clinical pharmacist to lead pharmacy services in its Better at Home programa subacute hospital substitution service delivering multidisciplinary care in patients homes.
As Senior Clinical Pharmacist youll work with a supportive team to optimise medication safety conduct reviews educate patients and carers liaise with community providers and contribute to personalised care planning. Youll also lead service development mentor junior staff and drive safe effective medication use in the community.

- Ability to demonstrate leadership and decision making skills
- Problem-solving and risk assessment abilities and the ability to use initiative
- Excellent communication time management skills with the ability to prioritise plan and organise your work and deliver to deadlines
- Ability to work effectively in multidisciplinary teams
Qualifications & Experience required
- Registered pharmacist in Australia
- Relevant post-graduate qualification in clinical pharmacy or leadership and management highly desirable
- Extensive hospital pharmacy experience in a senior leadership position with a demonstrated understanding of contemporary practice in hospital pharmacy services highly desirable
- Experience in leading and coaching highly effective team
