Job Title: Pharmacist

Specialty: Outpatient Inpatient and Community Health Pharmacy Services

Place of Work (City/State): Chinle Comprehensive Health Care Facility U.S. 191 & Hospital Drive Chinle AZ 86503

Duration: 3-months contract with the possibility of a 3-months extension

Start Date: October 2025

Shift Schedule: 10-hour shift

Shift Times:

  • 6:45 AM 5:45 PM
  • 7:00 AM 6:00 PM
  • 8:30 AM 7:30 PM
  • 9:00 AM 8:00 PM

Work Days: Monday to Friday including weekends and holidays must be flexible to rotate shifts.

License / Certifications:

  • Active unrestricted pharmacist license from a U.S. State Territory District of Columbia or Puerto Rico
  • Doctor of Pharmacy (Pharm.D.) or equivalent
  • Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certifications

Minimum Qualification / Experience:

  • 6 months of outpatient pharmacy experience in a hospital setting
  • Cultural awareness and understanding of American Indian/Alaska Native culture

Responsibilities:

Pharmaceutical Care Services:

  • Provide outpatient inpatient and community pharmacy services.
  • Fill and dispense prescriptions and over-the-counter medications accurately.
  • Counsel patients on drug use storage effects and interactions.
  • Review and evaluate medication regimens for appropriateness and compliance.

Clinical and Administrative Duties:

  • Under general supervision of the Chief of Pharmacy perform duties aligned with standard care protocols.
  • Ensure continuity of patient care and availability of qualified replacements when needed.
  • Adhere to CHCCHF safety standards and reporting requirements.
  • Participate in Joint Commission standards compliance and internal audits.
  • Submit incident reports for safety medical and performance issues as needed.

Interdisciplinary Collaboration:

  • Work with physicians PAs NPs and other clinical staff.
  • Provide drug therapy recommendations and assist in clinical decision-making.
  • Support transitions of care discharge counseling and public health education.

Documentation and Compliance:

  • Maintain accurate patient records in compliance with HIPAA and IHS standards.
  • Follow infection control and quality assurance protocols.
  • Meet performance standards in patient care documentation and professional conduct
