Investigates assesses and plans interventions to help patients cope with social emotional economic and environmental problems.
Essential Functions:
Interviews and assesses patients and/or patients family caregivers and/or legal representatives.
Determines prioritizes provides and/or arranges for needed internal and external services/interventions.
Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote monitor and evaluate compliance with patients treatment plan.
Assists with discharge planning and processes. Identifies appropriate resources including transportation housing healthcare and social/spiritual services and provides referrals as part of the discharge plan.
Performs Medicaid screenings as indicated for NHP.
Completes psychosocial history and summary (or update) within 72 hours of admission.
Provides psychosocial assessments written treatment and discharge plans that reflects the overall treatment objectives for patients.
Performs social work activities maintaining accountability for the delivery of quality social work services.
Plans and conducts daily therapy group sessions and programs with individual and family sessions as needed in accordance with the inter-disciplinary treatment plan and daily priorities of patients and needs of Behavioral Health Services.
Attends and participates in designated treatment team meetings on all matters concerning the patient and/or family.
Assists in the maintenance of a safe and therapeutic milieu.
Completes routine paperwork within prescribed time frames and efficiently documents on individual progress notes all pertinent and meaningful observations and information.
Adheres to the general expectations for all employees in order that the program functions in an effective manner to meet the treatment needs of patients.
Demonstrates knowledge of and a commitment to upholding the RRHS corporate compliance plans and their department specific compliance plan.
