About CLAREMATRIX:CLAREMATRIX CM is a nonprofit organization providing effective and compassionate treatment recovery and prevention services for alcoholism and substance use to individuals families and the community. Headquartered in Santa Monica CA CLAREMATRIX maintains multiple facilities in the Southern California region.

Since 1970 CLAREMATRIX has provided quality substance use and mental health treatment to men women and youth in southern California.

The following is representative of the duties responsibilities and requirements of the job but is not intended to be an exhaustive list.

Summary

Under the supervision of the Detox Program Manager a licensed healthcare professional and with a narrow latitude for independent judgment and decision this position works closely with the treatment team to administer care to the participants in a 24/7 detox and recovery facility. The LVN helps to ensure clients are safely monitored throughout detoxification and all facets of the day-to-day operations are run efficiently.

Shifts: Weekend Day Swing and NOC

Essential Duties and Responsibilities

  1. Patient Care

    1. Provide nursing care to patients undergoing detoxification including monitoring vital signs assessing withdrawal symptoms and reporting changes in patient condition.

    2. Support oversight of participant day-to-day operations and services.

    3. Conduct timely and comprehensive bed checks according to schedule and in accordance with program guidelines.

    4. Ensure clients are safely monitored throughout detoxification.

    5. Assist management team and treatment team with client supervision and service delivery.

  1. Testing and Documentation

    1. Ensure regular drug screens are conducted as directed by management ensuring that tests are logged appropriately.

    2. Complete incident reports as required.

    3. Assist in maintenance of program records and files.

    4. Maintain accurate and timely documentation of patient care in electronic health records (EMR).

    5. Provide clear and concise shift reports to ensure continuity of care.
  1. Safety and Compliance

    1. The LVN must adhere to all state and federal regulations including California Board of Vocational Nursing standards and prioritize patient safety and dignity in all aspects of care.

    2. Adhere to all safety rules and regulations.

    3. HIPPA Compliance: Secures participant information and maintains strict participant confidence by completing and safeguarding medical records and ensuring participant information is kept confidential.

    4. Professional Behavior: Strictly maintain professional boundaries following established limits and guidelines for appropriate behavior with participants ensuring a safe and ethical environment by clearly separating the professional role from personal interactions and protecting the clients privacy and dignity. Boundaries typically include but are not limited to personal disclosure limits physical touch restrictions and clear communication about the professional nature of the relationship.
    5. EnsurehealthandsafetyregulationsareupheldtoCLAREMATRIX standards OSHA CAL-OSHA and all regulatory requirements.
  1. Medication Administration
    1. Inventory Management: May monitor medication stock levels medication inventory organize medication storage and report low stock or discrepancies to supervisors.
    2. Prepare administer and document all medications according to physician orders and facility policies.
    3. Monitor patients for potential side effects or adverse reactions to medications.
    4. Educate patients on purpose potential side effects and proper use of prescribed medications.
  1. Billing for Medication Monitoring
    1. All notes must be completed in real time or at least by the end of shift to expedite billing.
    2. Submit UA results in MyEvovle to assist in billing process.
    3. Ensure health and physicals and TB test administration are reflected in the charts for billing to process timely and accurately.
  1. Teamwork and Collaboration
    1. Communication: Collaborate with nurses therapists and other healthcare providers to communicate participant changes needs updates and progress. Respond to staff and participants in a professional and courteous manner.
    2. Collaborate with cross-functional and multidisciplinary teams including but not limited to Department Heads Counselors therapists case managers and medical professionals to ensure comprehensive participant care.
    3. Participate in staff meetings one-on-one meetings with supervisors and other meetings as required.
    4. Maintainknowledge understandingand application ofCLAREMATRIXsmissionvisionandphilosophy.
    5. Education and Training: Participate in ongoing training team meetings and certifications to maintain eligibility for the role.

  1. Qualifications
    1. Must be a graduate of an accredited LVN program.
    2. Must have a current and unrestricted California LVN license.
    3. Must have a minimum of 1-2 years of experience in detox or substance abuse treatment is preferred.
  1. Knowledge Skills Competencies and Experience
    1. Experience in a detox or recovery center setting preferred.
    2. Knowledge of detoxification protocols and withdrawal management for alcohol and substance abuse.
    3. Strong assessment and clinical skills with the ability to respond quickly to changes in patient condition.
    4. Excellent communication organizational and time-management skills.
    5. Effective verbal and written communication and interpersonal skills.
    6. Organized and detailed oriented.
    7. Self-motivated and able to effectively and accurately follow directions and carry out job duties and responsibilities.
    8. Thrive in a fast-paced environment.
