Job Description
Overview
We are seeking a dedicated Medical Social Worker (MSW) to join our Covenant Home Health team. This is a PRN (as-needed) position offering flexible hours based on the needs of the patients and your availability.
In this role you will collaborate with the physician and other healthcare professionals to address the social and emotional aspects of patients health concerns. You will play an integral part in developing individualized care plans documenting clinical progress supporting families and connecting patients with community resources.
Additional responsibilities include participating in discharge planning in-service programs and offering expert consultation to Covenant Home Health Agency staff.

Responsibilities
- Understands and adheres to Agency policies/procedures.
- Assists the physician and other team members in understanding the significant social and emotional factors related to the patients health problems.
- Participates in the development of the patients plan of care as requested by the Director of Nursing.
- Conducts educational groups related to aging and geriatric population including Alzheimers and Dementia education.
- Completes clinical documents and progress notes for the patients electronic medical record in a timely manner.
- Assists the client and/or POA with appropriate community resources.
- Participates in Agency Care Conferences and QAPI program.
- Acts as a consultant to other Agency personnel.
- Provide intensive and/or continuing counseling to patients in evidence of crisis or need.
- Observes confidentiality and safeguards all patient information.
- Serves as a resource person to employees patients physicians and other allied healthcare providers.
- Maintains contact with community support groups and provides professional expertise as required.
Qualifications
- Licensed Medical Social Work (MSW) with a degree from an accredited MSW program.
- A minimum of One (1) year experience in Home Health preferred.
- Demonstrated ability to exercise initiative and independent judgment.
- Strong collaboration skills with the ability to engage and gain cooperation from diverse individuals to achieve shared goals.
