Job Description
Summary of Duties
To provide a full range of Social Work services to patients and families on the Short Term Complex Support and Transitional Care units of the Veterans Program. This includes supporting patients and families to meet their emotional and practical needs from admission until discharge completion of psychosocial assessments counselling locating and arranging community resources consultation and collaboration with individuals and teams. Candidates must have proficient skills with admission discharge and transition planning.
Qualifications/Skills
- Masters Degree in Social Work
- Member in good standing of the Ontario College of Social Workers and Social Service Workers
- Experience working in a hopsital setting (preferably 2 years); geriatric and end of life experience preferred
- Experience with the health care system discharge planning and community resources
- Cognitive support experience required
- Excellent assessment interviewing and problem solving skills
- Excellent written and oral communication mediation time management and organizational skills
- Superior ability to manage multiple tasks and set priorities in a fast-paced environment
- Familiarity with issues of capacity and consent
- Strong negotiation and conflict resolution skills
- Ability to be a team player as well as self-directed
- Commitment to person centred care
- Commitment to student education
