The Role
The Care Coordinator role is responsible for management and coordination of client care via diverse means of support and providing a connection point at any stage of recovery. The primary means include telephonic chats emails as well as direct client interaction for walk ins. This role involves spending adequate amount of times with clients by providing information and resources assessing client needs crisis intervention services as well as facilitating warm hand offs to internal and external programs. This role will also triage client needs and ensure timely follow up for needed services.

Answer calls and inquiries professionally ensuring a comfortable and positive experience
Listen to clients story determine best resources or programs and provide adequate follow up
Crisis intervention as identified during calls/situations
Utilize warm hand offs to other programs or clinicians to ensure a streamlined process
Educate potential clients about substance use and related issues as appropriate for their situation
Is knowledgeable about The Council on Recovery services and activities as well as other local and/or national community level treatment providers substance abuse service agencies and support groups
Cross-trained in welcome center venue and other tasks to ensure coverage
Following up with clients and ensuring closed loop referrals from external partners
Qualifications
Bachelors Degree required
Masters Degree in social work psychology counseling or related field preferred or relevant licensure such as LPC LCDC LMSW; or 5 years of relevant experience
Bilingual preferred
Experience with substance abuse evaluation counseling and case management
Understanding of ASAM and DSM 5 diagnostic criteria for substance use disorder and mental health disorders
Working knowledge of community resources and experience in developing relationships
Problem solving skills and ability to take initiative
