Job Summary:
This Registered Nurse (RN) position requires a minimum of two years of experience in a hospital home health or hospice setting. The RN will provide skilled nursing care case management and hospice care to patients focusing on patient assessments medication administration treatment planning and collaboration with the interdisciplinary team. Strong communication and collaboration skills are essential.
