Job Description
- Active Registered Nurse license (if RN role is selected)
- Licensed Practical Nurse/Licensed Vocational Nurse license (if LPN/LVN role is selected)
- Certified Nursing Assistant (CNA) license (if CNA role is selected)
- BLS certification (if CNA or LPN/LVN role is selected)
- Completion of annual TB risk assessment
- COVID-19 vaccination or exemption
- Background check (including criminal drug screen and others listed)
- Education verification
- Employment verification
- Fingerprint results (NM Caregivers Criminal History)
- HHS OIG Exclusions Search
- Influenza vaccination (during flu season)
- MMR vaccination or positive titer
- New Mexico Employee Abuse Registry check
- OFAC SDN-Sanction Check
- SAM/EPLS (SAM Exclusion Search)
- TB screening (TB Gold or TB Skin Test)
- Varicella vaccination or positive titer
- Vision screening (color vision exam)
- 4-year degree (if RN role is selected)
- Monthly HHS OIG Exclusions Search.
- Compliance with all listed background checks and screenings within specified timeframes.
- Adherence to TriCores Acceptable Use Policy Agreement and Compliance Confidentiality Form.
- Seasonal influenza vaccination requirement.
