Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients of all ages with various health conditions. The RN will perform patient assessments administer medications develop treatment plans and collaborate with the interdisciplinary team. Strong communication and collaboration skills are essential. The role involves on-call responsibilities and occasional weekend coverage.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and management.
- Treatment planning and implementation.
- Collaboration with interdisciplinary healthcare team.
- Providing skilled nursing care case management and hospice care.
- Maintaining accurate patient records and documentation.
- Responding to patient needs and concerns.
- On-call duties (1-2 nights per week).
- Occasional weekend coverage.
Required Skills & Certifications:
- Clinical assessment medication administration and treatment planning skills.
- Minimum two years of hospital home health or hospice care experience.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and personal automobile for field work.
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- On-call requirement: 1-2 nights per week.
- Occasional weekend coverage required.
- Block scheduling is utilized.
- Patient ratios are 1:1.
- 48-hour workweeks are not approved.
- Short-term acute care/Level II trauma hospital setting.
Scheduling:
- Block scheduling is used.
