Schedule: Day Shift 5x8-Hour (08:30 AM 5:00 PM)

We are seeking an experienced and compassionate Physical Therapist (PT) with strong outpatient neuro rehab experience for a 13-week contract at our Brea CA location. The therapist will manage a caseload of primarily neurological patients including post-stroke vestibular and Parkinsons. This is a highly collaborative and dynamic rehab environment.
Responsibilities:3 years of PT experience required (neuro experience preferred)
First-time travelers welcome if minimum experience is met
Epic charting experience required
Block scheduling available per manager discretion
Locals not accepted
Pending license not accepted
RTO must be submitted with profile

Perform initial evaluations assessments and reassessments for outpatient neuro rehab patients
-
Develop individualized plans of care based on evidence-based treatment techniques
-
Provide direct therapeutic interventions focused on neurologic conditions mobility balance and cognitive retraining

Educate patients and caregivers on home exercise programs safety and injury prevention
-
Accurately document treatments and outcomes in Epic EMR
-
Collaborate with interdisciplinary teams to ensure optimal patient outcomes
-
Manage a caseload of approximately 40 patients with up to 8 9 patients per day

Participate in team meetings case discussions and discharge planning

3 years of recent outpatient PT experience required
-
Neuro-specific experience highly preferred
-
Must have experience with documentation evaluations discharge summaries and reassessments

Proficiency with Epic charting system
-
Must maintain schedule flexibility and excellent time management skills
-
Strong interpersonal and communication skills to work with diverse patient populations
-
Graduate of an accredited Physical Therapy program (DPT preferred)
-
Minimum of 3 years of Physical Therapy experience with exposure to neurologic rehabilitation strongly preferred
-
First-time travelers accepted only if they meet the full experience requirement

Current and active California PT license required at time of submission

No pending licenses accepted

Comprehensive patient evaluations treatment planning and goal setting
-
Post-stroke rehab concussion Parkinsons disease vestibular dysfunction and cognitive disorders
-
Experience with pediatrics (e.g. torticollis) is a plus
-
Documentation and billing for Medicare B Medicaid commercial and workers comp
-
Patient education and injury prevention
-
Time management with 45-minute sessions across a full-day schedule
-
Ability to manage and prioritize a high patient volume with quality care
-
Experience in rehabilitation hospitals and outpatient clinic settings
-
Knowledge of medications home exercise programs and adaptive techniques
