Job Summary:
This role is for a Physical Therapist Assistant (PTA) providing home health services including initial evaluations and follow-up visits. The PTA will manage a caseload of 5-10 patients daily traveling within a 75-mile radius of the assigned clinic. The position requires a minimum of one year of adult home health experience and adherence to a point-based productivity system.

Responsibilities:
- Provide physical therapy services to patients in their homes.
- Conduct initial evaluations and follow-up visits.
- Maintain accurate and timely documentation of patient progress and treatment plans.
- Travel to patient homes within a designated territory.
- Attend periodic office meetings.
- Manage a caseload of 5-10 patients per day achieving a weekly point goal.
- Collaborate with the interdisciplinary team.
Required Skills & Certifications:
- Current Physical Therapist Assistant license.
- Minimum one year of adult home health experience.
- Current CPR/BLS certification.
- Reliable transportation valid drivers license and agency-required liability insurance.

Special Considerations:
- Mileage reimbursement at the IRS standard rate (patient home to patient home only).
- Guaranteed hours are not provided; however support may be available for contractors facing financial hardship due to low patient census.
- Weekend work may be required depending on location.
- On-call requirements vary by location and will be discussed during the interview.
- Travel up to 75 miles from the home clinic is required.
- Local assignments are accepted.
Scheduling:
- Standard schedule is typically Monday-Friday 8:00 AM - 5:00 PM.
- Some locations may utilize a 4x10-hour schedule (Friday-Monday).
- 13-week contract assignment.
- No weekend or on-call rotations are expected for most positions.
