Job Summary:
This Licensed Practical Nurse (LPN) position involves providing and coordinating home nursing care supervising home health aides and LPNs/LVNs and adhering to physician orders agency policies and procedures. The role requires extensive travel within a specified territory managing a caseload of 5-10 patients daily and utilizing electronic medical record systems.

Responsibilities:
- Provide skilled nursing care in home health settings according to physician orders and agency protocols.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct patient assessments administer medications and develop treatment plans.
- Maintain accurate and timely documentation using electronic medical record systems (HCHB).
- Manage a caseload of 5-10 patients daily performing various visit types (Start of Care Wound Care Follow-up).
- Travel to patient homes within a 75-mile radius of the assigned clinic.
- Attend periodic office meetings.
- Comply with all agency policies and procedures.
Required Skills & Certifications:
- Current LPN/LVN license in the state of employment.
- Minimum of one year of experience in adult home health or hospice care.
- Proficiency in HomeCare HomeBase (HCHB) electronic medical record system.
- Current CPR/BLS certification.
- Valid drivers license car insurance and reliable transportation.
Preferred Skills & Certifications:
- Experience with Oasis (for RNs PTs and OTs).
Special Considerations:
- Extensive travel required within a 75-mile radius; mileage reimbursed at the IRS standard rate. Commute to and from the first and last patient is not billable.
- Weekend requirements vary by location; discussed during the interview process.

- Most schedules are standard 5x8s (Monday-Friday) but some locations may have a 4x10s (Friday-Monday) Baylor shift schedule.

- The standard work schedule is Monday-Friday with the possibility of a 4x10s (Friday-Monday) schedule in certain locations.
