Job Description
We are hiring a Licensed Practical Nurse I for one of our clients in Trenton NJ.
Job Description:
- LPNs process referrals by assessing the information with attention to CA Checklist and the recommended information requested.
- Collaborate with referring stakeholders.
- Ensure accuracy adequacy and completeness of referral by requesting all missing documentation necessary for medical clearance.
- LPNs will review referral and determine if patient meet criteria for the Short-Term Care Facilities (STCF) Diversion bed or County hospital based on Insurance and psychiatric presentation.
- If the LPN determine that the referral is as tate referral the coordinator will write a brief synopsis summarizing the reason for referral. The LPN will assess (labs Medication and Legal document to ensure compliance with NJ mental health statutes etc..
Work hours:
- 3pm to 11:30pm
- Lunch period: 1hr (unpaid).
