Job Description
We are seeking a compassionate and skilled Speech Therapist to join our dynamic home health team. The ideal candidate will provide high-quality speech language and swallowing therapy services to patients in their homes helping them achieve their communication and functional goals.
Responsibilities:
- Conduct comprehensive assessments of patients speech language cognitive-communication and swallowing abilities.
- Develop individualized treatment plans tailored to patients needs and goals.
- Provide therapy to improve speech articulation language skills voice fluency and swallowing function.
- Educate patients and families on strategies to enhance communication and manage swallowing difficulties.
- Collaborate with interdisciplinary team members including physicians nurses and other therapists to ensure holistic patient care.
- Document patient progress and maintain accurate records in compliance with regulatory standards.
- Adhere to home health safety protocols and infection control measures.
Qualifications:
- Current Colorado licensure as a Speech-Language Pathologist.
- Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA) preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and manage a flexible schedule.
- Valid drivers license and reliable transportation for home visits.
- CPR certification required.
