Home Health Physical Therapist (PT)
A Home Health Physical Therapist is needed in Manhattan NY. Manhattan offers a vibrant urban setting with world-class cultural attractions diverse neighborhoods and excellent public transportation. This position is with a mission-driven home health organization committed to delivering high-quality personalized care to patients in their homes.
Salary and Benefits
- $100000 $120000 annually
- 25 PTO days in year one
- 6 paid holidays

Responsibilities
- Provide in-home PT following physicians care plan
- Achieve 30 points/week (6 patients/day)
- Assess treat and adjust individualized care plans
- Monitor progress document outcomes adjust as needed
- Educate family/caregivers on rehabilitation techniques
- Collaborate with care team for comprehensive patient care
- OASIS experience preferred
Requirements
- Current NY State PT license
- Graduate of an accredited PT program
- Home health or physical therapy experience
- Strong communication and interpersonal skills
- Knowledge of current treatment practices and clinical expertise
