Pay Rate: $90000 - $95000/yr.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provides clinical leadership across assigned communities by implementing assisted living and memory care protocols
- Performs and documents resident assessments and Individualized Service Plans; delegates hands-on care as needed
- Coordinates clinical services with physicians pharmacists and third-party providers
- Conducts skill evaluations of care team members; performs daily performance rounds
- Leads recruitment hiring training scheduling and performance management for clinical staff
- Ensures compliance with state regulatory standards and maintains readiness for health inspections
- Investigates and resolves resident and family concerns; provides superior customer service to promote retention
- Assesses prospective residents to determine compatibility with community care capabilities
- Supports transfers and move-outs with care team coordination
- Tracks trends and reports clinical quality data to identify risks and improve outcomes
- Oversees medication management programs to ensure resident safety
- Completes incident investigations and ensures timely documentation
- Maintains accurate records and ensures compliance with all applicable regulations
- Actively engages in emergency preparedness and response across communities
- Collaborates cross-functionally with Sales teams Life Enrichment Directors and other disciplines
- Supports community culture through person-centered care and team morale building
- Operates and maintains equipment inventory and supplies in line with protocols
- May be assigned additional duties based on resident needs and operational requirements
Minimum Requirements
- College degree required; graduate of an accredited LVN/LPN/RN program
- 23 years of related experience preferably in assisted living or memory care
- Strong foundation in geriatric nursing emergency care and first aid
- Current unencumbered RN or LVN/LPN license in the state of Texas
- Current CPR certification and Continuing Education credits per state requirements
- Valid Drivers License
- Proficient in electronic medical records and Microsoft Office suite
- Excellent written and verbal communication skills
- High level of organization attention to detail and professional demeanor
- Flexible availability across shifts days and weekends
