Title: Discharge Planner for Inpatient Behavioral Health

Salary: $64000-$74000

Location: Cushing OK (onsite)

Job purpose

  • This position is responsible for supporting insurance verification pre-certification and certification as well as navigating the necessary systems for patients requiring placement and discharge. This position also collaborates with the community to ensure the appropriate discharge planning of all patients.


As PMCs Discharge Planner your focus will be on the 3Cs; Care Community and Compliance.

Care Provide the best possible patient care

  • Educates patients and their families on the signs and symptoms of illness to prevent rehospitalization and improve disease management skills.
  • Provides assessments treatments and case management to patients and families.
  • Participates in discharge planning arranges for follow-up care and develops mechanisms for the exchange of appropriate information with sources outside the hospital.

Compliance Ensure the program operates within all regulations

  • Maintains and participates in performance improvement activities within the department.
  • Responsible for any insurance verification/certification needed.
  • Initiates pre-certification and recertification of insurance for all registered patients who require it.


Community Become a wonderful addition to your local community

  • Serves as a resource person to healthcare professionals concerning psycho-social problems and community resources.


Requirements


  • Bachelors Degree or higher in Social Work or Nursing (RN or LPN) License required
  • Two or more years of previous experience in all aspects of social service or nursing with an emphasis on psychiatric work.
  • BLS
  • Handle with Care training (will be provided if not current)
  • Basic computer knowledge
  • Strong interpersonal skills
