Hours- Monday through Friday 7:30-4:00 no evenings no weekends no holidays.
Collaborates with leadership to support practice transformation and population health efforts. Acts as a liaison between clinical staff and management focusing on coordinating the clinical operations of the practice Panel Management Patient Education and Health Coaching. Supports care management efforts within the team through process improvement workflow redesign providing assistance with training and delegating to other members of the team.
Position Responsibilities
Collaborates with Practice Leadership to conduct Daily Huddles and Weekly Care Team meetings in which the team will review practice population health data to improve care and achieve practice goals.

Under direction of Clinician coordinates Transitions of Care:
Collaborates with Lead Medical Assistant to:
Performs daily regulatory testing as required and documents accordingly to ensure that the clinical equipment is accurate and compliant.
Required Experience:
Minimum 6 months experience in a physician practice.
Knowledge of minor surgical procedures and sterile technique.
Multitasker with high organizational skills.
Required Education:
Graduate of approved LPN School of Nursing.
Required Training/Certification/Licensure:
Current NJ LPN License.
BLS Certification from American Red Cross or American Heart Association.
