Job Description Summary
To coordinate the activities of the Movement Disorders clinic. The RN will coordinate the outreach and patient education activities for the clinic. The nurse will also be a resource for Movement Disorders patients in the clinic as directed by the movement disorders attendings.Entity

Job Description
- Assist provider with clinic visit on an as-needed basis
- Attend clinic appointments for advanced patients as needed
- Assess needs of patients at these clinic appointments
- Set up interdisciplinary clinic (scheduling mailing materials pre-clinic assessments educational materials summaries coordination of referrals relaying results)
- Provision of telehealth (needs assessment for patients)
- Meet with other providers case managers and social workers and collaborate on common patients and referral workflows
- Collaborate with interdisciplinary team regarding shared patients and referrals
- Community outreach and educational event planning
- Facilitate communication between patients and providers and schedule appointments as needed
- Submission of referrals
- Assist with orders for devices/DME
- Assist with patient care/COE tasks as needed/delegated
Additional Job Description
Bachelors degree in nursing required and three years related nursing experience. Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Some positions require certification as a generalist in a related specialty area by the American Nurses Association (ANA).
