PT Home Health Shift: 5X8 M F 8am - 4pm Daily Caseload per tech: 5-6 units Years of experience REQ: 1 First-timers accepted: (Y/N) Possibly after interview CERTS: BLS BJC policy: Every other weekend required Is on-call REQ (Y/N) What is the req No Will clinician float within scope meet facility needs No Floating Req to Sister Facilities: (Y/N): No Open to accommodating block schedule NA Locals accepted: (Y/N) Yes Pending State License accepted: (Y/N) No RTO Restrictions: No more than 7 days; provide requests and BJC will determine if able to approve or choose another day if possible. Guaranteed Hours: 40 hrs. Special Requirements: Department - BJC Home Health in Glen Carbon Il. consists of a Multidiscipline Team Geriatrics and middle age no pediatric patients. Patient Dx - Ortho cardiac neuro pulmonary Diabetes other. Case management skills independent time manage and organizational skill patient centered goals supervising PTA Team Player knowledge professionalism communication skills flexibility positive attitude provide excellent quality care.
