Hourly Rate: $48-$60 DOE & Seniority
Collaborate with residents families and our care team to develop and communicate care plans (PT OT treatment protocols etc.)
Be the go-to support system for residents answering their questions and guiding them through every step of their care journey
Monitor changing resident conditions and coordinate timely appropriate interventions
Lead morning stand-up meetings and keep care teams aligned and informed
Oversee admissions discharges and care referrals in a way thats smooth and supportive
Complete required documentation ensure timely charting and manage utilization reviews
Coordinate care with other providers case managers financial services and insurance providers
Serve as a dependable point of contact for residents families nurses physicians and third-party partners
RN license preferred (LVN/LPN acceptable)
Bachelors degree in Nursing or Social Work
Minimum 2 years of clinical nursing experience preferred
Familiarity with Medicare Medi-Cal Medicaid and PCC software
Strong communication and problem-solving skills
A heart for people and a brain for organization
Ability to work independently manage priorities and think on your feet
A respectful professional approach to all interactions from residents to teammates
Have experience with admissions/discharges and care transitions
Know your way around scheduling and electronic charting systems
Can lift up to 50 lbs (yep sometimes we all lend a hand!)
