HOME MANAGER The Home Manager (HM) is responsible for providing leadership management regulatory compliance supervision and program plan implementation Individuals receiving waiver services. The HM provides direct care to Individuals during scheduled shifts manages the staff at the home coordinates meetings and establishes relationships with the Individualized Support Teams functioning as a liaison between the teams and LEL. Upholds LELs vision mission and values.
Reports to:Director of Community Living Supervises:Direct Support Professionals (DSPs) Position Title will: - Available to workdays evenings. overnights and/or weekends based upon the needs of the home.
- Provides on-call services when needed
- Supervises DSPs including training leading meetings scheduling and communicating.
- Work closely with the Director of Community Living Director of Individual Finance and Human Resources to assure that the files of Direct Support Professionals and Individuals are maintained in accordance with applicable laws and LELs Policies and Procedures Manual.
- Work closely with the Individualized Support Team (IST) in assessing the Individuals risks and assuring a plan is in place to address the Individuals identified risks.
- Process quality assurance complaint incident and change of status reports and notify the appropriate parties with the reports.
- Prepare investigate and submit all Incident Reports for their assigned caseload.
- Work with Individuals IST to prepare all Individuals goals.
- Ensure that each of the following are reviewed and agreed upon by the IST at each quarterly meeting:
- The appropriateness of services
- Risk assessments and current risk plans
- The appropriateness of ISP goals
- Progress towards ISP/PCP goals
- The appropriateness of current medication as well as compliance
- Recent and upcoming medical appointments
- Health and safety of the Individual
- Review and ensure that all documentation required by applicable law and the LEL Maintenance of an LEL Individuals Personnel File Policy are maintained in the Individuals Blue Book and copies are forwarded to the central office for the master file.
- Be available during emergencies.
- Exemplifies promotes and supports valued roles in the lives of all LEL Individuals.
- Demonstrates competencies as a Direct Support Professional by providing assistance to Individuals as they pursue the Good Life
- Any other duties requested by LEL.
Qualifications: - Must have a high school diploma post-secondary degree preferred.
- Two years management experience preferred.
- Two years experience serving people with developmental physical or mental disabilities preferred.
- Knowledge of intellectual disabilities mental health human behavior gentle teaching person-centered training quality of life and relationships.
- Each HM must meet state and company requirements.
- Experience and skills necessary to perform services listed above.
- Strong interpersonal and relationship building skills.
- Customer service orientated.
- Satisfies all requirements under applicable law.
- A positive and pleasant attitude.
- Ability to work independently and meet deadlines.
- Strong organizational skills and close attention to detail.
