As our Lead Support Worker in the Buller Court Mental Health Team youll be right at the heart of supporting customers with complex mental health needs helping them build the skills and confidence to live more independently in the community.

Typical day as a Lead Support Worker (known by us as Coordinator)
Supporting working age adults with a variety of needs including mental health substance misuse physical disability learning disability and autism always with a person-centred approach.
Starting your shift with a handover making sure youre up to speed on any changes in our customers wellbeing risks or support needs.
Managing your own diary and caseload delivering support sessions focused on practical life skills like cooking shopping budgeting maintaining tenancies and helping customers move on to independent accommodation.
Carrying out risk assessments support planning goal setting and regular reviews to keep everything on track and tailored to each customer.
Enjoying plenty of cuppas chats and catch-ups with colleagues and external partners because teamwork and collaboration are at the heart of what we do.
Leading on client involvement organising customer meetings and making sure every customer has a voice in our service because everyone deserves to be heard.

You bring
A genuine passion for supporting people to live their best lives working collaboratively and always keeping an eye for detail.
Experience in creating person-centred support plans and helping colleagues work to those plans making sure everyones on the same page.
Confidence in coordinating and assessing customer referrals ensuring the right support reaches the right people at the right time.
The ability to work independently stay calm under pressure and bounce back from challenges with resilience and positivity.
A vehicle insured for business use so you can get from A to B (dont worry well pay your mileage!).

Job details
We aim to be flexible when agreeing a working pattern with our colleagues but you will be needed to work a mixture of earlies lates and your fair share of weekends and bank holidays.
Youll manage your own caseload and diary to suit the needs of customers on your caseload giving you a good balance of autonomy and support.
Confidently use technology for creating and updating support plans completing online learning and collaborating with colleagues.
Youll need an Enhanced DBS check done and we pay for that
