Job Description
The Grand Rehabilitation and Nursing at South Point in Island Park is now hiring aCase Manager/Discharge Planner!
Schedule: Monday to Friday day shift
The Case Manager / Discharge Planner plays a key role in coordinating comprehensive discharge planning and case management services for residents. This position ensures residents have a smooth transition to home assisted living or other appropriate settings and that their discharge needs are met in a timely and efficient manner.
Key Responsibilities:
- Oversee and coordinate care for residents in alignment with individual care plans
- Conduct comprehensive assessments and participate in care plan development and reviews
- Effectively communicate with HMO providers and ensure appropriate authorizations are in place
- Monitor resident health status manage medications and ensure proper documentation to comply with DOH and CMSguidelines.
- Communicate regularly with families physicians and interdisciplinary team members
- Provide training and guidance to care staff on clinical procedures and best practices
- Support regulatory compliance and quality assurance initiatives
- Assist in transitions of care and discharge planning
- Participate in Utilization Review meetings and ensure optimal LOS is utilized for the benefit of the patient
- Monitor outcomes and re-admissions to support continuous quality improvement
Requirements:
- Bachelors degree in Social Work Nursing or a related field.
- Prior experience of 2 years or more in Discharge Planning or Case Management in a SNF.
- Knowledge of Medicare Medicaid and managed care plans.
- Strong clinical assessment critical thinking and communication skills.
- Familiarity with NYS DOH regulations.
- Proficiency with EMR systems and Microsoft Office.
