In this role you ll be at the heart of creating personalized care plans that help participants regain mobility independence and continue thriving at home. Collaborating with the interdisciplinary team (IDT) to assess and develop personalized care plans.
Evaluating mobility levels and physical conditions to create customized therapy plans.
Delivering hands-on therapy with therapeutic exercises gait training and ambulation support.
Developing dynamic individual and group exercise programs to support rehabilitation and maintenance.
Providing high-quality physical therapy services always aligned with physician orders and care plans.
Conducting thorough assessments to identify key areas for improvement and recommending follow-ups.
Referring participants to additional services and resources when needed for optimal recovery.
Educating participants and their families empowering them with the knowledge to succeed in their rehab journey.
Managing and maintaining therapy equipment to ensure safety and efficiency.
Collaborating with staff in nursing homes skilled nursing facilities and assisted living settings to ensure comprehensive rehab care.
Occasionally conducting in-home assessments and providing treatments to participants in their own space.
Monday through Friday: 8:00 am 5:00 pm
100% On-site at the facility with occasional travel to patients homes SNFs or other healthcare facilities.
Required:
Master s degree (or higher) in Physical Therapy.
Valid PT License in California (CA).
AHA Basic Life Support (BLS) certification.
Proof of a valid California driver s license reliable personal transportation a good driving record and auto insurance (as required by state law).
At least 1 year as a Physical Therapist.
Must be located within 10 miles of Downtown LA.
