Position Summary:
The Medication Reconciliation Pharmacy Technician will assist the pharmacy nursing and medical staff in obtaining an accurate home medication list. The technician will interact with patients and caregivers physician offices pharmacies long term care facilities and/or any other reasonable means to obtain an accurate medication list. The technician will update the medication list in the medical record to reflect accurate medications dosages frequencies and indications. Care will correspond to patients unique medical needs and will be appropriate to the patients agenewborn pediatric adolescent adult or geriatric. In accord with Department and institutional polices/procedure and with the rules/regulations enacted by the State Board of Pharmacy they will support the Departments logistic clinical technical and clerical functions.

Minimum Qualifications:
Required: High School graduate or equivalent. Minimum 1 year experience working in a pharmacy or administering medications in a healthcare setting. PTCB (Pharmacy Technician Certification Board) certification or Licensed Practical Nurse
Preferred: Completion or graduate of a pharmacy technician training program or licensed practical nursing program. 3 years experience in either of the above areas
