We are seeking a dynamic highly motivated Harm Reduction Navigator who demonstrates our values of compassion accountability responsiveness and empowerment; to our clients families and each other.
The Harm Reduction Navigator is reporting to the Manager Housing & Outreach Hamilton this position will assist in providing intake case management and navigation supports as well as carrying out day-to-day administrative services. Within a multi-disciplinary team you will actively provide support and therapeutic services to individuals experiencing homelessness and living with acute substance use and related health and mental health issues. Services are based on a residential harm reduction approach to service delivery.
This position is a unionized full-time permanent position. The rate of pay is $29.89 per hour. This position is 40 hours per week.
Key Responsibilities:
Work with individuals in Wesleys Harm Reduction Housing Program to support stabilization in order to identify housing needs.
Manages a caseload of individuals that are using illicit opioids and safe supply.
Liaise with Community Partners to develop housing and treatment pathways.
Provides feedback on policy updates to ensure the program remains current and mitigates risk.
Create case plans to reflect housing goals recovery pathways life skills education employment planning and after care as well as informal counselling and safety plans.
Assist with making connections in the community based on the residents wants and needs.
Assist residents with their move into accommodations and provide neighborhood orientation to build social capital.
Provide support and referrals for appointments in the community.
Maintain regular effective communication with administrators community partners and physicians to ensure an integrated approach that will provide the best support possible for residents to maintain housing.
Conducts regular visits with residents to provide supports specific to life skills daily living skills conflict resolution crisis intervention and prevention budgeting assistance financial literacy community integration and community building grocery shopping cleaning social rec etc.
Maintain appropriate and accurate record-keeping practice as per expectations of Wesley.
Evaluate the program on an ongoing basis and recommend service delivery changes.
Support with on-call rotation coverage.
Other duties as assigned.
What you will bring:
Minimum of five years of relevant practical experience with the target population and post-secondary degree from a recognized educational institution in Human Services Social Work or related field.
Preference will be given to candidates with registration and member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
Must have experience working and counselling individuals with mental health and addictions issues and people experiencing homelessness and providing non-judgmental supports to them.
Must be able to meet travel expectations of the position; a valid Drivers license and access to a vehicle is an asset.
Lived experience is an asset.
