About the Role
As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Dandenong and surrounding areas in the south east.
The successful candidate will be responsible for processing referrals assisting clients in commencing services and continuously reviewing and adjusting care plans to suit their evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.
This is a permanent full-time role based in our Dandenongoffice with hybrid work from home arrangements and travel to clients homes surrounding suburbs when required.
