As a Community Health Patient Navigator you will play a crucial role in the Emergency Department Syphilis/HIV/HCV Screening Program (EDSP) supporting routine opt-out testing and treatment for syphilis HIV and hepatitis C virus (HCV). Working under the guidance of the Program Manager you will provide patient education outreach and linkage to care ensuring that patients receive the support they need.
In this grant-funded position you will collaborate with emergency department staff local healthcare providers and community organizations to coordinate referrals advocate for routine STI testing and establish effective follow-up systems. Your work will help normalize STI screening and improve health outcomes for diverse and underserved populations.
