- Provide community navigation support to the newly awarded Improving access to primary care for multicultural communities project

DPV Health has recently been awarded the Improving access to primary care for multicultural communities project on behalf of the North Western Melbourne PHN (NWMPHN). The purpose of the project is to address the needs of multicultural communities within Hume LGA for navigation support and equitable service experience from the primary care system.
The project aims to increase engagement with the healthcare system for multicultural communities via a Community Navigator Program leveraging already established networks and working collaboratively with multicultural organisations local councils and community health providers to disseminate information on primary health services.
This role has specific accountabilities aligned with the project as per the agreement with the NWMPHN.
This project is a 10-month project commencing Mid-August 2025 and ending mid- June 2026.
Skills And Experiences
Bring your great everyday!
You will be a motivated individual with experience in engaging within multicultural communities within the Hume LGA. You have experience and knowledge of codesigning practices and incorporating insights and feedback into project tools and communications.
Importantly you have excellent interpersonal and communications skills as well as theability to build relationships with community members and leaders.
Most of all you are passionate about helping multicultural communities to access local primary health services in order to improve their health outcomes and live better lives.
The fine print
- Successful candidates will be required to undergo a National Police Check International Police Check (if applicable) hold a valid WWCC.
- Full driver license and travel between DPV Health and other partner sites will be required.
