Job Description

Job Summary

The APRN and Nurse Coordinator CF Program in association with the supervising physician assumes responsibility for providing comprehensive care at an advanced practice level to CF Program children with craniofacial differences. The APRN and Nurse Coord CF Program will provide comprehensive assessment and management for such children in two distinct but coordinated settings: the craniofacial multi-disciplinary program at Nicklaus Childrens Hospital housed within the Brain Institute and inpatient referrals. APRN multi-disciplinary team functions are to include but are not limited to the coordination of craniofacial team clinical assessments and patient care including the composition of multidisciplinary summary treatment plans facilitation of diagnostic procedures such as nasopharyngoscopy evaluations with speech and language pathologists and performance of inpatient consultations. Nurse and CF Program Coordination functions include navigation referral source building and communication community outreach Referring PCP and patient/family Team Summary communication registry data entry oversight of quality improvement activities and healthcare transition.

Job Specific Duties

  • Develops implements evaluates monitors manages and modifies a plan of care that prescribes interventions to attain expected outcomes.
  • Performs comprehensive and problem focused history and physician examinations.
  • Orders and interprets appropriate laboratory tests radiological studies and consultations.
  • Designs and performs healthcare transition activities adhering to State Grant protocols and requirements.
  • Meets quality improvement activities in accordance with State Grant requirements.
  • Proactively performs patient navigation consistent with a high performing CF Program.
  • Develops participates and participates in community and patient / family support functions.
  • Performs procedures as demonstrated through competency.
  • Accountable for maintaining current expertise and skills in order to facilitate a high quality of clinical performance and to promote professional growth.
  • Ensures proper communication between physicians and families. Serves as a advocate and educator and provides families with support and community resources.
  • Orders and/or prescribes medications for inpatients/outpatients.
  • Participates in other necessary activities associated with patient care (writing daily progress notes coordinating clinical care discharge planning etc.)
  • Writes orders monitors/alters drug therapies and performs other procedures with education and experience within established protocol in collaborative relationship with a Physician(s).
  • Recognizes need for physician consultation and guidance as indicated within experience and scope of practice.
  • Maintains a high level of critical thinking and time management skills. Completes documentation accurately and in a timely manner of 24 hours.
  • Acts as a preceptor for APP students.

Minimum Job Requirements

  • Masters degree in nursing or Doctor of Nursing Practice (DNP) from accredited school
  • APRN Valid State of Florida Licensure - maintain active and in good standing throughout employment
  • Certification by the American Nurse Credentialing Center (ANCC) American Academy of Nurse Practitioners (AANP) or Pediatric Nursing Certification Board (PNCB)- maintain active and in good standing throughout employment
  • American Heart Association BLS - maintain active and in good standing throughout employment
  • Level II Background Check
  • Successful completion of Credentialing by NCH Medical Administration

Knowledge Skills and Abilities

  • Pediatric Nurse Practitioner experience and certification preferred.
  • Able to relate cooperatively and constructively with patients families and co-workers.
  • Ability to communicate effectively verbally and in writing.
  • Able to maintain confidentiality of sensitive information.
  • Ability to problem solve to formulate a plan of care and evaluate the patients response to care.
  • Ability to interpret adapt and react calmly under stressful conditions.
  • Ability to perform expert specialized skills that are required for specific specialty area.
