What we are looking for:
Competency in providing follow up and discharge planning services and support to individuals served : 1) Individuals in psychiatric inpatient settings 2) Assessment Evaluation and Facilitation of services 3) Crisis Intervention 4) Individual empowerment/self-advocacy 6) Community and Service Networking 7) Community Living skills and support 8) Linking to Vocational Educational career support and self-development resources 9) Communication and quality Documentation skills 10) initial contact during hospital admission within 72 hours as well as follow up appointments within 7 days post-discharge for adults and children served by CNS Healthcare.
Compensation Range:
The starting pay for this position is $64400 - $74400 for limited licensed candidates and $70150 - $80150 annual salary for fully licensed candidates and is based on non-discriminatory factors such as skills and experience.

Essential Functions:
Assessment
- Complete Intake Assessments Develop Complete and Implement the required Person-Centered Planning (PCP) process Periodic reviews Housing placement applications various court documents ALS contract completion as appropriate.
- Respond to the needs desires and interests of individuals served utilizing formal and informal assessment practices.
- Establish a collaborative relationship with individuals using a variety of effective communication strategies and skills.
- Assess individuals needs and develop treatment plans to address significant deficits in their ability to perform life tasks and live successfully in the community.
Facilitation of Services/Consultation
- Match prevention intervention and resolution techniques to the particular circumstances of each individual as required.
- Identify and use effective advocacy strategies to address and overcome diverse challenges facing individuals (e.g. human rights legal administrative and financial).
Self-Advocacy/Education/Community Awareness
- Identify and utilize the resources and support necessary to assist individuals in reaching goals relative to career and educational pursuits.
- Match specific supports and interventions to the unique needs of individuals and recognize the importance of friends family and community relationships.
- Enhance the ability of the individuals to lead a self-determining life by providing support and information necessary to build self-esteem and assertiveness to make decisions that would improve their quality of life.
- Assist individuals in identifying and gaining access to formal and informal supports available in his/her community.
Medication Management and Monitoring
- Assist in the coordination and monitoring of medication administration for individuals served as required.
Customer Service/Time Management
- Complete all required documentation completely accurately and in a timely manner.
Team Building/Professional Development
- Lead or participate in at least one process improvement initiative (task force committee CNS staff and/or person served activity etc.) each evaluation period.
- Demonstrate competency in Key Dimensions for this classification per attached sheets.
- Support and conduct oneself in a manner consistent with customer service expectations.
- Demonstrate and actively promote an understanding and commitment to the mission and values of CNS Healthcare through performing behaviors consistent with the Organizational Code of Ethics.
- Maintain a working knowledge of applicable Federal State and Local laws and regulations the Corporate Compliance Program Code of Ethics as well as other policies and procedures in order to ensure adherence e in a manner that reflects honest ethical and professional behavior.
Qualifications:
Required
- Masters Degree in Social Work or Counseling; two or more years of experience with adults in a mental health setting preferred;
- Michigan license required; LLMSW LMSW LLPC LPC
- Valid State of Michigan Drivers License
- Transportation or automobile accessibility and proof of current auto insurance required
- Must have experience working with computers various software and data bases
Preferred
- Demonstrate knowledge and understanding of mental illness medications used in treating psychiatric disorders and knowledge and experience with other interventions used to treat psychiatric disorders.
- Maintain a high level of interpersonal and problem-solving skills provide excellent customer service orientations skills in working effectively with various levels of internal agency personnel outside customers and community groups with an ability to communicate effectively in multiple professional environments i.e. Probate Court State Hospital facilities State and County Agency personnel Community agencies Individuals served and their family members.
- Identify areas for self-improvement pursue necessary education/training resources and share knowledge with others.
