The Transfer Center Nurse functions as the primary liaison for organizations and providers seeking access to services or patient transfers to Concord Hospital acting as a crucial component of a multidisciplinary team. Additionally they are responsible for the management of patient placement and flow within the organization which calls for prioritizing needs and triaging bed availability with bed requests. The Transfer Center Nurses use established guidelines and criteria together with a review of medical records to determine the placement of all admissions and transfers.
Education
Graduated from an accredited Nursing Program required
Bachelor of Science Degree in Nursing (BSN) is strongly preferred
Certification Registration & Licensure
Current and active New Hampshire Nurse license
American Heart Association Healthcare Provider BLS required on hire
American Heart Association Healthcare Provider ACLS preferred
Experience
Minimum five years of Acute Care Nursing experience
Previous supervisory/leadership experience desired
Demonstrated aptitude for multitasking and organizing as well as excellent phone etiquette and customer service skills.
Demonstrate leadership ability and management skill including proficiency leading diverse teams in a highly complex matrix management structure.
Respected and acknowledged for the ability to remain composed under pressure and for a professional approach to solving problems while attempting to achieve a win-win outcome.
Serves as an example of the service behaviors expected of Concord Hospital.
Excellent interpersonal communication skills both written and verbal.
Excellent relational coordination and emotional intelligence are displayed by: maintains cordial and cooperative relationships; constructively handles conflict; assists in problem identification and provides solutions; spearheads initiatives for ongoing improvement.
Demonstrates a readiness to acquire expertise and proficiency in software applications pertinent to the operations of the Transfer Center.
Ability to work a flexible schedule including weekends holidays and evening and night tasks is required.
Responsibilities
Monitors current institutional capacity to assess impact patient flow resources and capacity management.
Serves as a liaison to assist providers in finding and utilizing the right Concord Hospital services.
Coordinates transfers to Concord Hospital from local regional and international locations for patients who are currently affiliated with Concord Health System or who need higher level of care.
Coordinates communication between Concord Hospital providers and referral providers.
Communicates with referring facilities regarding transfer and bed assignment.
Actively participates in daily capacity huddles with department leadership nursing supervisors and providers to identify appropriate mitigation strategies that optimize safe patient intake and patient care resource demands.
Develops and promotes long-term relationships with referring facilities and providers.
Monitors evaluates and improves quality of care and services provided.
Advocates on patient behalf in obtaining access to appropriate services.
Actively engages in the daily implementation of strategic goals as they relate to capacity management patient flow and service-line development.
Interprets and supports policies procedures and safety standards to facilitate daily operations.
Supports the overarching goals of the Concord Hospital Health System.
Performs other duties as assigned.
