Job Description

Primary Purpose
The Care Manager in the Dean of Students office provides non-clinical intervention resources and support to Pacific students on a broad range of issues affecting their retention and persistence at the University. This position has a primary role in the coordination and follow up of students referred to the Pacific Care Team and intervenes to provide short-term case management to those students who have been identified as at risk for attrition due to personal social or complex academic challenges; areas of concern can include but are not limited to: mental and emotional health physical health crime victimization financial hardship social adjustment and other needs. The Care Manager collaborates with campus partners to ensure all students referred to the Care Team receive effective outreach comprehensive interventions and follow up services to support their retention and success. This position is physically located on the Stockton campus and provides virtual support to the Sacramento and San Francisco campuses.

Essential Functions
Management of student referrals and an ongoing caseload of students; ability to prioritize student issues and needs and keep timely documentation and case notes of progress in Maxient Ellucian Advise and other university platforms. Implement proactive case management strategies to intervene and provide support for assigned students including those with significant or compounding academic financial or personal challenges. Assess student concerns and behavior using appropriate risk rubrics; use assessment to inform outreach approaches intervention strategies and to identify escalating concerns. Provide timely and compassionate support for students experiencing personal crises including medical emergencies urgent mental health concerns housing and food insecurity and bereavement needs; serves as an advocate for these students with faculty and other campus constituents. Participate in the office of the Dean of Students weekly Care Team meetings provide updates on critical student cases interventions and identify outstanding student needs and/or escalating concerns. Exercise independent judgement to assess and respond to urgent student needs including initiating welfare checks coordinating emergency housing needs and escalating critical concerns to appropriate campus partners and university leadership. Complete required reporting and referrals to the Behavioral Intervention Team Student Conduct office Title IX and office of Diversity Equity and Inclusion. Work closely alongside Student Conduct Title IX and Victim Advocate to provide appropriate support for student complainants and student respondents. Demonstrate cultural sensitivity and competence in working with diverse students staff families and community members. Provide education and guidance to faculty and staff on recognizing and responding to students in distress; assist in the training of student RAs as well as student life groups to share the best practices in student support. Build and maintain collaborate campus relationships with key campus partners to coordinate comprehensive and effective support for students. Remain knowledgeable about community resources and assist students in connecting to qualifying community support. Understand and follow FERPA HIPAA and other relevant laws and regulations; uphold the highest levels of confidentiality. Perform other duties as assigned. This position serves as Campus Security Authority ( CSA ) as outlined by the Clery Act. CSAs have federally mandated responsibilities to report crimes that are reported them. Minimum Qualifications
Bachelors Degree Three (3) years related professional experience working with a diverse population.

Preferred Qualifications
Masters Degree preferred Five (5) years related professional experience in working with a diverse student population. Demonstrated intercultural competence and the ability to successfully work with diverse constituencies. Demonstrated understanding of social justice issues and sensitivity to concepts related to diversity access equity and multiculturalism in higher education; demonstrated commitment to promote and enhance diversity and inclusion. Demonstrated ability to be flexible and adaptable in a dynamic fast-paced environment; skilled at managing competing priorities and responding effectively to changing circumstances. Demonstrated experience in executing a referral/case management system; knowledge of Student Information Systems including Maxient and Ellucian Advise is desirable. Demonstrated case management skills including the timely recording tracking and follow up of cases. Experience working with students with complex needs and/or limited participation. Effective communication skills and demonstrated ability to express ideas clearly and concisely both verbally and in writing especially the ability to simplify detailed information and articulate complex ideas in a manner that is easily understood. Knowledge of federal and state laws concerning confidentiality of student records. Experience and sensitivity in working with people of diverse backgrounds and cultures. Demonstrated experience in advancing social justice equity and inclusion in a university setting. Ability to engage and integrate culturally responsive practices and knowledge in their work.
