Job Description

Reporting to the Hospice Medical Director under the direction of the Chief Medical Officer the Granite VNA Physician is responsible for determining patient eligibility for Hospice services. Collaborates with internal and external providers.


Essential Functions

  • Determines patient eligibility for Hospice service in accordance with program acceptance criteria and the Conditions of Participation.
  • Certifies and re-certifies patient eligibility for the Hospice Program in conjunction with the attending physician on a timely basis.
  • Keeps timely accurate and complete medical records as required by Granite VNA comply with guidelines and requirements established by Medicare Medicaid and all other applicable payors (e.g. patient care documentation and medical billing processes) and maintains patient confidentiality and privacy in accordance with state and federal law including but not limited to the Health Insurance Portability and Accountability Act (HIPAA). This includes Certification of Terminal Illness and Recertification of Terminal Illness documents.
  • Participates in Granite VNAs Hospice interdisciplinary group (IDG) to provide medical consultation and physician representation support at patient case conferencing meetings as requested.
  • Consults with primary physicians or specialists as needed regarding the patients medical condition. Assumes responsibility for the general medical needs of Hospice patients to the extent that the attending physician does not meet the needs serving as attending physician as needed and when requested by the patient or their representative. Works with the attending physician to encourage their fulfilling of their responsibilities.
  • Serves as a liaison between the attending physician and the Hospice Team by obtaining/sharing information about the patients plan of care on an ongoing basis.
  • Evaluates patients medical status in the home outpatient and inpatient settings based on a visit schedule.
  • Works within the hospice provider team to develop an on-call schedule for management approval.
  • Provides Face-to-Face hospice visits in the community facilities or Hospice House as mutually agreed upon.
  • Understands the regulations and standards related to the Medicare Hospice Conditions of Participation.
  • Actively participates in the Hospital and Respite facility joint committees by providing input to them during scheduled meetings.
  • Attends Patient Care Conferences as requested.

  • Understands and demonstrates the agencys safety policies and procedures.
  • Demonstrates flexibility in the role by accepting other duties as assigned for which qualified.
  • Performs job duties in a safe manner in order to prevent injury to yourself and others.


Expected Hours of Work

The employee will be scheduled to work based on the needs of the Hospice program. Work assignments may include weekends or holidays.

Requirements
  1. Medical degree (MD or DO is required)
  2. Current NH medical license
  3. Board certified in Internal Medicine or Family Practice
  4. One to two years of medical physician experience including pain and symptom management experience
  5. Current Drug Enforcement Administration (DEA) license
