ChristianaCare HomeHealth is seeking a Full Time LPN Nurse Liaison. The successful candidate will support New Castle County DE working out of our North Wilmington Office; work day shift hours 8am-4:30pm and every other weekend day some holidays may be required based upon departmental needs.
Duties and Responsibilities:
- Develop Relationships with referral sources providers and other health care professionals.
- Pre-admission evaluation and education of patients identified for home health to identify care needs available resources and validate information needed to ensure safe transfers across the care continuum.
- Perform ICD-10 coding of referrals.
- Coordinate with branch office manager to schedule start of care and supplies for services ordered by the provider as well as communication with referral sources and providers.
- Identify patients appropriate for specialty programs and telehealth.
Requirements:
- LPN
- Previous case management or discharge planning preferred.
