Job Highlights:

TheTMS Occupational Therapist must be considered aQualified Professional (QP)and have at least2 years of experience working with adults with a mental health diagnosis and/or substance use disorder.

This Opportunity:

The primary responsibilities of the Transitional Management Services Occupational Therapist are to provide clinically-based supervision and guidance to both staff and the individuals they support enhancing quality of life through services that enable individuals to achieve their personal dreams and goals. In addition to Qualified Professional duties this role includes delivering occupational therapy services to support individuals in achieving greater independence and functional ability in their daily lives.

What Youll Do:

  • Provide initial and ongoing assessments of individuals functional abilities occupational performance and therapeutic needs.

  • Evaluate the effectiveness of the Person-Centered Plan (PCP) and progress made by individuals receiving services.

  • Participate in interdisciplinary team meetings and case reviews offering clinical input and recommendations for PCP modifications.

  • Develop and provide input on goals for the person centered plan based on assessment findings.

  • Deliver direct OT interventions to improve skills in areas such as self-care sensory processing fine/gross motor coordination and adaptive functioning.

  • Recommend and train individuals and staff on the use of assistive technology and environmental modifications.

  • Maintain accurate and timely documentation of therapy sessions and progress in accordance with professional and regulatory standards.

  • Identify opportunities for community connections and the development of natural support networks.

  • Educate individuals and staff on available community resources and facilitate access to these supports.

  • Use a range of communication strategies to build effective relationships with individuals families staff and other stakeholders.

  • Collaborate with interdisciplinary teams to ensure coordinated and person-centered service delivery.

  • Ensure service requests align with eligibility continued stay and discharge criteria.

  • Conduct clinical reviews and ensure documentation meets service requirements for authorization and billing.

  • Prepare and submit required reports evaluations and documentation. Review and sign off on clinical records as needed.

  • Provide clinical guidance and expertise to direct support staff in the implementation of Person-Centered Plans (PCPs) and support strategies ensuring alignment with occupational therapy goals and best practices.

  • Collaborate with staff to model and reinforce therapeutic techniques that promote individuals independence functional skills and engagement in meaningful activities.

  • Offer consultative support by observing service delivery identifying areas for improvement and recommending evidence-based strategies tailored to individual needs.

  • Participate in team meetings to share insights on individuals progress and functional abilities helping staff adjust approaches as needed.

  • Assist in developing and delivering training sessions or informal coaching to enhance staff understanding of occupational therapy principles and person-centered practices.

  • Serve as a resource for staff questions related to adaptive equipment sensory supports and environmental modifications.

  • Promote a team-based approach by fostering open communication mutual respect and shared problem-solving among all team members.

  • Maintain certification in all required agency state and federal training.

  • Demonstrate knowledge of emergency procedures and participate in on-call coverage.

  • Work with supervisors to assess and enhance the quality and effectiveness of services provided.

  • Ensure compliance with all agency policies procedures and applicable regulations.

  • Follow service definition guidelines for all services provided.

  • Complete other responsibilities as assigned by the supervisor.

  • Driving and travel may be required.

Education Were Looking For:

Bachelors: Occupational Therapy (Required) Masters: Occupational Therapy

Certifications Were Looking For:

Drivers License (Valid) - USA Occupational Therapist (OTR/OTA-L) - State Board of Occupational Therapy Qualified Professional (QP) - Monarch-DSM

Experience Were Looking For:

Experience working with adults with a mental health diagnosis and/or substance use disorder 2 Years Required

Schedule:

Monday-Friday (9:00am-5:00pm)
